Maybe the pink thing isn’t hurting the belcher. Maybe that’s just it burping, unrelated to the activities of the pink thing. You know: wake up, fart, scratch, belch. It fits right in.
@Me: But Belcher looks like he’s not used to that kind of bodily activity. The little exclamation in the 5th panel kind of suggests that. And its eyes are bulging out! (Well, bulging out more than usual.) That can’t be normal! And lookit all that smoke!
This is a creature that routinely is fed meals the size of entire (okay minus the head) people for the purpose of vomiting them back up. It does undergo quite drastic changes that way. I don’t know about the smoke though. We’ll have to wait and see.

The descender’s category is misc, which doesnt make sense since descender is the only comic in it… there, changed it
